§ 2-28. Alternate employee group medical insurance plan.

(a)

The alternate employee group medical insurance plan provided by health maintenance organization AV-MED of Louisiana shall be offered for enrollment by choice to all active/retired parish employees and their dependents effective September 1, 1988.

(b)

The present proportionate share of premium contributions now paid by the employee/employer shall be the same amount for whichever plan (AV-MED or New York Life - only one) he/she chooses or elects for himself/herself and dependents.

(c)

All employee/dependent life and dental benefits will continue to be covered under the present contract with New York Life Insurance Company until expiration December 31, 1988, regardless of the medical plan chosen.

(d)

All premium contributions from both plans (New York Life Insurance or AV-MED) paid by the employee/employer shall be deposited into Plaquemines Parish Government Employee Group Insurance Program Account #84-004-0 with Delta Bank and Trust Company, official depository of the Plaquemines Parish Government.

(e)

Drafts drawn on the Plaquemines Parish Government Employee's Group Insurance Program Account #84-004-0 with Delta Bank and Trust Company, payable to New York Life Insurance Company and AV-MED of Louisiana, shall be honored by Delta Bank and Trust Company, after approval of said drafts by the President of the Plaquemines Parish Government, the Parish Director of Administration, and the Parish Accounting Supervisor.
